Feedback forms

You can use the forms below to contact us about the following types of feedback.

  • Enquiry
    A request for information or suggestion made to the department about its services, service delivery, products, decisions or actions. An enquiry is a question that requires a response from the department.
  • Compliment
    An expression of satisfaction made to the department about its services, service delivery, products, decisions or actions. Compliments do not usually require a response from the department but may be used to record feedback.
  • Complaint
    An expression of dissatisfaction made to the department about its services, service delivery, products, decisions or actions of the department or its staff that is not resolved at the point of service. Requires a response from the department in accordance with departmental policy.
